Getting Started

All of our providers are accepting new patients, so feel free to give us a call to schedule a well visit or sick appointment at a time that works for you. If possible, please print and fill out the new patient forms found in the "Forms and Services" section to save time when you come to the office, and arrive at your first appointment 15 minutes prior to your appointment time.

Expecting parents or families with kids thinking about transferring to our practice are welcome to set up an appointment (at no cost) to meet with any of our physicians informally, during which we can answer any questions you have. You can find our prenatal and new family interview forms on our site - feel free to fill them out before you arrive at the office to streamline the process.

Appointment Scheduling and Routine Visits
We have weekday hours ranging from 8:00 AM - 4:30 PM as well as Saturday morning hours and selected evening hours, and all of our sick appointments are scheduled for the same day you call (pending availability). Routine Well Visits for health supervision and immunizations are scheduled at the following ages:

  • Newborn Visit 1-3 Days after Hospital Discharge
  • 2 Weeks
  • 1 Month
  • 2 Months
  • 4 Months
  • 6 Months
  • 9 Months
  • 12 Months
  • 15 Months
  • 18 Months
  • 24 Months
  • 30 Months
  • 3 Years
  • Annually until age 18 or High School Graduation (with routine booster immunizations at age 4-6, 11-12, and 16-18)

Medical History and Records Transfer
Please come to the office prepared to tell us about your child's medical illnesses, hospitalizations, and surgeries. Bring a list of all of their medications and exact doses (including inhalers and associated equipment), or the actual medication containers if you have any questions. Please share family and social history issues with the provider at the first visit and update this information as changes occur.

Please contact your child's previous physician to transfer records to our office prior to their appointment if at all possible. You can complete a Records Transfer Form (found on this site) to expedite this process - ask our office staff if you have questions. It is essential that we have a copy of your child's immunization record. If you do not have a copy or cannot obtain it from your prior physician, you can usually obtain a copy from your child's school.

Billing and Insurance
A current insurance card must be presented at each office encounter. Payment is expected at the time service is provided. If there are special circumstances regarding payment, please discuss with your provider. A charge will be made for all appointments made but not kept, and for all returned checks.

All co-payments will be collected before your child's visit.

Due to the increasing demands of managed care, it is imperative that our office be notified of any specialist care or emergency room visits before they occur. If an emergency occurs on a weekend, the doctor on call must be notified before you go to the ER, unless a life-threatening situation exists. You must also notify our office on the next business day. We cannot and will not backdate referrals. This policy is in effect for your financial benefit and protection.

Each child seen must have his or her own appointment time (we are happy to see multiple siblings during the same appointment if you schedule them all at the time you call - in fairness to other patients waiting to be seen, we cannot accommodate requests to see other children once the provider is with you).

We accept many different insurance plans - if you have questions about whether your insurance is accepted by our group, please call the office to speak to the office manager.
